The Ontario Government has mandated that as of September 22, 2021, individuals must be fully vaccinated and provide proof of their vaccination status to access certain settings. This requirement falls upon establishments including restaurants, gyms, and indoor recreational and entertainment centres including Perplexity Escape Games.
Individuals aged 12 and over, unless exempt, can provide either a paper or digital copy of the vaccination receipt that demonstrates they are fully vaccinated (see above for definition). It must include name, date of vaccination and product name (brand of vaccine).
Note: Proof of a negative COVID-19 test will not be accepted as an alternative to proof of full vaccination.
A vaccination receipt includes any of the following:
There are some allowable exemptions from the proof of vaccination requirement such as children under age 12 and patrons with a medical exemption.
Patrons who provide a written document, completed and supplied by a physician (designated as “MD”) or by a registered nurse in the extended class (designated as “Registered Nurse (Extended Class)”, “RN(EC)”, “Nurse Practitioner” or “NP”) stating that the individual is exempt for a medical reason from being fully vaccinated against COVID-19 and the effective time-period for the medical reason do not have to show proof of vaccination. Patrons with a medical exemption are required to present identification and a written document stating the individual is exempt for a medical reason.
With regards to proof of vaccination, we will not keep or share any information received from customers. Vaccination receipts and ID's will be viewed and verified in person, but no personal details will be retained. However, customer information provided to us via our Participation Waiver and/or Contact Tracing Forms, is confidentially kept on file, but will not be shared unless where required by law.
